US Passport – adult 18+

Evidence of US Citizenship AND a Photocopy of the front and back, if applicable, must be submitted with your application.  Photocopy must be made in black ink and be on 8 1/2" X 11" plain white paper.

You must submit ONE of the following.  Your evidence must be an original or certified copy only.

(A certified copy is any document that has the seal or stamp of the official issuing authority)

• U.S. Passport (Expired Passport may be used)
• U.S. Birth Certificate that meets the following requirements:
• Issued by the city, county, or State of birth
• Lists your full name, date of birth, and place of birth
• Lists your parent(s)' full names
• Has the date filed with registrar's office within one year of birth
• Has the registrar's signature
• Has the seal of the issuing authority
• Consular Report of Birth Abroad or Certification of Birth
• Certificate of Naturalization
• Certificate of Citizenship

Evidence of Identity AND a Photocopy of the front and back of the Identification card must be submitted with your application. Photocopy must be made in black ink and be on 8 1/2" X 11" plain white paper.

You must present ONE of the following IDs that includes a photo to the Acceptance Agent.

• Valid or expired, undamaged U.S. Passport book or U.S. Passport card
• In-state, fully valid Driver's License with photo
• Certificate of Naturalization
• Certificate of Citizenship
• Government employee ID (city, county, state, or federal)
• U.S. Military or Military Dependent ID
• Current (valid) foreign Passport
• Matricula Consular (Mexican Consular ID)
• U.S. Permanent Resident Card (Green Card)
• Trusted Traveler ID (including valid Global Entry, FAST, SENTRI, and NEXUS cards)
• Enhanced Tribal Cards and Native American tribal photo IDs

You must appear in person before an Acceptance Agent at an Acceptance Facility.

The USPS Acceptance Office requires scheduled appointments. 

Submit and present all your Documents, along with your Photos, and pay your Fees at your Acceptance Facility appointment.

Fees must be submitted in correct payment forms with the Application.

Please verify all required fees and provide separate payments for both the Application & Application Execution Fees.

Completed, but UNSIGNED, Passport Application – Form DS-11 filled out in BLACK ink.

If printed from your computer, it must be printed on single sheets. 

DO NOT USE DOUBLE-SIDED PRINTING!
