All Applications for a Child’s US Passport (under 16) must be submitted in person with the minor child and both parents/guardians, along with Proof of US Citizenship, Parental Relationship & Identity, Passport Photo and the required Fees at an Acceptance Facility.
Evidence of US Citizenship AND a Photocopy of the front and back, if applicable, must be submitted with your Child’s application.
Photocopy must be made in black ink and be on 8 1/2″ X 11″ plain white paper.
You must submit ONE of the following for your Child. Your evidence must be an original or certified copy only. (A certified copy is any document that has the seal or stamp of the official issuing authority)
Evidence of Identity AND a photocopy of the front and back of the Identification Card for each Parent/Guardian must be submitted with your Child’s Passport Application. Each Photocopy must be made in black ink and be on 8 1/2″ X 11″ plain white paper. Each Parent/Guardian must present ONE of the following IDs to the Acceptance Agent.
You must submit documentation to show the parental relationship that lists the Parents/Guardians of the child applying for the passport.You must submit ONE of the following. Your evidence must be an original or certified copy only.
The Child and both parents/guardians must appear in person before an Acceptance Agent at an Acceptance Facility.
If one parent/guardian cannot go with the child to apply for the passport, they can give permission by completing Form DS-3053.
The parent that cannot appear in person with the child must have the Statement of Consent form signed in the presence of a Notary Public for submission with the child’s passport application.
Include the following with the Child’s Passport Application:
Completed, but UNSIGNED, Passport Application – Form DS-11 filled out in BLACK ink.
If printed from your computer, it must be printed on single sheets.
DO NOT USE DOUBLE-SIDED PRINTING!
Fees must be submitted in correct payment forms with the Application.
Please verify all required fees and provide separate payments for both the Application & Application Execution Fees.

If you would like to check the status of the minor's Application, the Department of State website offers status updates after 7-10 days.
You must submit ONE (1) 2″ X 2″ Color Passport Photo of your Child that meets all US Passport guidelines for correct head-size, position, and total photo size.
