US Passport renewal – adult 18+

You must submit your most recent US Passport with your Renewal Application.The following must ALL apply to your US Passport in order to Renew:

• Passport must be undamaged
• You were age 16 or older when it was issued
• It was issued within the last 15 years
• It was issued in your current name (or you can document your name change with an original or Certified Copy of your Marriage Certificate, Divorce Decree, or signed Order of the Court)

You must provide a Certified Copy of the Legal name change Document as evidence of the name change if your current name is different from the name on your most recent U.S. Passport.

• Marriage Certificate
• Divorce Decree
• Court-ordered name change document

(Your name change document will be returned to you in a separate mailing)

Fees must be submitted with the Renewal Application.

• Passport Book $130
• Passport Card $30
• Passport Book & Card $160
• Make Check or Money Order payable to "US Department of State."
• Please print your Full Name and Date of Birth on the front of your payment.

Compile your items for Mailing:

• Completed Renewal Application
• Your most recent Passport
• Name change Document (if applicable)
• Passport Photo
• Fee with your information printed on the payment

Mail your Application to one of the addresses below.If you live in the United States, you MUST use the United States Postal Service (USPS) to mail your Renewal Application and supporting documents via a Trackable delivery method.  

If you use UPS, FedEx, DHL, or any another company to deliver your application, the application mailing will be returned to you. 

If you live in Canada, you should use Canadian Post. Please do not use other delivery services such as UPS, FedEx, or DHL.
